Enrollment

Screening

The first step toward getting into treatment at The Haven is to fill out our screening form:

If you are filling the form out for yourself or someone who is not incarcerated please go here: The Haven's Online Screening Form

For incarcerated persons, please use the form found here:
The Haven’s Screening Form for Incarcerated Individuals.

(If the individual is in Salt Lake County Jail and is represented by the Salt Lake Legal Defenders Association, they will need to be screened and referred to us by an SLLDA social worker).

After our Admissions team has had a chance to review your form, they will contact you for a screening interview and determine your eligibility for The Haven.

Waitlist and Check-in

After you are approved for treatment at The Haven, you will be put on our waitlist for an available bed. While on the waitlist, you will be expected to check in with our Admissions team once a week to remain on our list. When availability opens for you, we will contact you to arrange intake.

Detoxification

The Haven does not offer detoxification services. We do require you to have at least five days of continuous sobriety at the time of admission. However, if you are experiencing symptoms related to withdrawal that could be a safety risk to yourself and others and could prevent you from participating in treatment, we will refer to a facility who provides these services. This will allow you the necessary time to withdraw in a safe environment until you are medically cleared to be admitted to
The Haven.

Intake

On the day of your admission to The Haven, you will be expected to arrive at the facility by the time arranged between yourself and our Admissions team. If you need assistance with transportation to get there, the Admissions team can help arrange that. Your treatment will begin with an assessment and an orientation to the program.
